Eligibility for Healthy Utah

At this time, only employees and their covered spouse, or dependent (Salt Lake City employees only) are eligible to participate; other dependents are not covered. All group health care plans under the PEHP umbrella, including Preferred Care, Advantage Care, and Summit Care plans, qualify for the Healthy Utah Benefit. myHealthyUtah FAQs

What is myHealthyUtah?
myHealthyUtah is your online account tool to access Healthy Utah. You can use your myHealthyUtah account to:

Review your past testing session results, including blood pressure, cholesterol, weight, and body composition.

  • Schedule or change testing session appointments.
  • See the date of your last testing session appointment.
  • Preview the rebates for which you are currently signed up.
  • Check the status of your rebate payment.
  • Sign up for Health Challenges, such as Maintain, Don't Gain.
  • View current news from Healthy Utah.

How do I create a myHealthyUtah account?
Simply follow these steps to create your FREE account:

  1. Visit myHealthyUtah.
  2. Create an account.

Note: You will need your PEHP ID number, so have your insurance or dental card handy!

 

How do I recover my password if I have forgotten it?
If you can't remember your password, you can go to our login page at www.healthyutah.org/myhu and click on Forgot Password. Enter your email address and select "Send my username and password HINT," or "Generate and send a new password." Our computer system will send your request as soon as possible.

What if I reset my password, but the automatic email never arrives?
Some email programs will think our automatic email is span or junk, and will put it in your junk folder. If this isn't the case, please notify healthyutah@pehp.org and we will assist you.

Can my spouse and I use the same e-mail address on both of our accounts?
No. If you ever need to retrieve your password HINT or have a new password generated and sent to you, your information is sent through e-mail.

Can I use my account to sign up my spouse for a testing session or submit a rebate for him/her?
No. Your spouse needs their own activated account to be able to sign up for a testing session, sign up for Health Challenges or track rebates.

Why has my account been de-activated?
There are several potential reasons:

  1. If you have changed your insurance plan within PEHP, your eligibility then changes from inactive to active with the onset of the new plan.
  2. If you dropped your PEHP medical insurance coverage for any reason, your account becomes inactive.
  3. Your agency may have elected to opt out of the Healthy Utah benefit.

My email address has changed. How do I update that on my account?
You can send your new email address to healthyutah@pehp.org, or you can call our office at 801-366-7300, or toll free at 855-366-7300.

Why can't I create a new account? The program says it can't find me in the system.
Please make sure to register your new account with your first and last name exactly as it is shown on your insurance card. Also, use your 13-digit PEHP ID number (17410000xxxxx).

I submitted my rebate at least three weeks ago, but I haven't received a check yet. What should I do?
You can check the status of your rebates by logging into your myHealthyUtah account and clicking on "Rebate History." The rebate status is updated as it is submitted for payment to PEHP. Once the status says "Paid," your check should arrive within three to four weeks.

Can I still use myHealthyUtah after I retire?
If you are not on medicare as your primary and PEHP as your supplemental insurance your myHealthyUtah account will remain active. You can attend a testing session every year and participate in the rebate program.


Rebate FAQs

What are rebates?
Healthy Utah wants to reward individuals for making changes that can improve health. Cash rebates are awarded to members who successfully accomplish specific criteria.

What rebates are offered?
Healthy Utah offers a "First Steps" and "Good For You" rebate. If you don't qualify for the "Good For You" rebate, you may qualify for an improvement rebate. Please download and print the First Steps and Good For You Rebate Form (PDF) or the Improvement Rebate Form (PDF).

Do I have to register for Healthy Utah before I can qualify for rebates?
Yes. You will need to register before you can complete your online Health Questionnaire.

How do I qualify for and participate in the rebate program?

  1. BMI Improvement Rebate
    To qualify: Your body mass index (BMI) must be 25 or greater. Calculate your BMI, or attend a Healthy Utah testing session.
    To earn the rebate: Download and print our Improvement Rebate Form (PDF).
  2. LDL Cholesterol Improvement Rebate
    To qualify: Your total cholesterol level must be 200 mg/dL or higher. A valid cholesterol result can be obtained from your primary care physician or a Healthy Utah testing session.
    To earn the rebate: Decrease your LDL cholesterol to your goal level.
    Required documentation: Download and print our Improvement Rebate Form (PDF).
  3. Blood Pressure Improvement Rebate
    To qualify: Your blood pressure would need to be greater than 120/80 mm/Hg to qualify.
    To earn the rebate: Working with your physician, set a goal blood pressure and successfully accomplish it.
    Required documentation: Download and print our Improvement Rebate Form (PDF).
  4. Diabetes Management Rebate
    To qualify: You must currently be diagnosed with type I or type II diabetes.
    To earn the rebate: You must lower or maintain your A1C at 7 or lower for the past year. The A1C tests reflects your blood sugar activity over a period of three months.

  5. Tobacco Cessation
    To qualify: You must currently use tobacco.
    To earn the rebate: You must be tobacco free for six months.

How long does it take to receive my rebate check?
Once we receive your rebate information, it can take up to six weeks for you to receive your check. Most checks arrive within a month, however. If it has been six weeks or longer since you submitted your rebate, please call our office.

Can my spouse participate in the rebate program?
Yes, if he/she is insured with PEHP.

 

Testing Session FAQs

What is involved in a testing session?
Healthy Utah offers testing sessions at various sites around the state. When you participate in a testing session, we will check your cholesterol, blood glucose, blood pressure, waist circumference, and weight. We then offer you one-to-one counseling and information on how to improve your elevated test results. Attending a testing session is the easiest way to sign up for the rebate program.

How long does a testing session take?
A complete session lasts about 20-30 minutes, depending on how busy we are. We generally schedule four people per half hour. If you have several questions or concerns, we are happy to spend extra time with you to ensure you have the information necessary to make informed and healthy choices.

Do I need an appointment?
Yes. An appointment is vital to ensure the quality of the health screening and to adhere to the schedule our members have planned on. We value your time and want to make sure we help you in a timely manner.

Can I attend a testing session if I am pregnant?
Yes. Although, your results may not actually reflect your non-pregnant measurement levels. Your cholesterol and body composition will increase. Nursing will also keep your cholesterol level elevated, so use your best judgment when making an appointment to be tested. You can participate in the First Steps rebate, and if you don't qualify for the Good For You rebate, you can participate in the Good For You, Good For Your Baby rebate.

Do I need to fast for the cholesterol or glucose tests?
You don't need to fast for a cholesterol test. However, to achieve a lower blood glucose, you may want to fast up to four hours before.

My cholesterol was normal at the last Healthy Utah testing session I attended. Should I have it tested again?
Medical guidelines recommend having your cholesterol checked once every five years if it is within desirable guidelines (total cholesterol less than 200). However, we will check your cholesterol annually.

If my blood pressure is elevated, do I have hypertension?
The American Heart Association refers to hypertension as a condition in which the resting blood pressure is consistently at or greater than 140/90 mm Hg (millimeters of mercury). Levels of pre-hypertension are 120-130 mm/Hg (systolic) or 80-90 mm/Hg (diastolic). A single elevated blood pressure measurement does not necessarily mean that you have hypertension or pre-hypertension, but it should not be ignored.

There are several reasons that multiple measurements are needed to diagnose hypertension. Instruments used to measure blood pressure are not always accurate, such as the automated cuffs available at pharmacies. Accuracy even varies among health care workers. There are also factors that can temporarily increase a person's blood pressure, such as taking over-the-counter cold medications or having an abnormally stressful day. For these and other reasons, any elevated reading found by someone other than your physician or their staff must be confirmed by your doctor.

Are the Healthy Utah staff registered nurses?
We are not nurses, but we are trained health professionals. Staff members either have or are working on Bachelor's degrees in Health Promotion and Education, Exercise and Sports Science, Nutrition, or other health-related degrees. Many of our team also have Master's degrees in health areas and other professional certifications.

Our staff members are required to pass stringent annual state certifications to perform cholesterol, glucose (blood sugar), and blood pressure tests. They are required to maintain their certification in First Aid and CPR.
