Eligibility for Healthy Utah
Healthy Utah is an employee health and wellness program available to agencies who have elected to offer a PEHP plan that includes this benefit. At this time, only employees and their covered spouses are eligible to participate; dependents are not covered.
All group health care plans under the PEHP umbrella, including Preferred Care, Advantage Care, and Summit Care plans, qualify for the Healthy Utah Benefit.

myHealthyUtah FAQs
What is myHealthyUtah?
myHealthyUtah is your online account tool to access Healthy Utah. You can use your myHealthyUtah account to:
- Review your past testing session results, including blood pressure, cholesterol, weight, and body composition.
- Schedule or change testing session appointments.
- See the date of your last testing session appointment.
- Track and submit the Physical Activity Rebate.
- Submit your completed weight loss, cholesterol, blood pressure, diabetes, and tobacco cessation rebates, with required documentation.
- Preview the rebates for which you are currently signed up.
- Check the status of your rebate payment.
- Sign up for Health Challenges, such as Colorful Choices and Maintain, Don’t Gain.
- View current news from Healthy Utah.

How do I create a myHealthyUtah account?
Simply follow these steps to create your FREE account:
- Visit www.healthyutah.org/myhu
- Create an account.
Note: You will need your PEHP ID number, so have your insurance or dental card handy!
How do I recover my password if I have forgotten it?
If you can’t remember your password, you can go to our login page a www.healthyutah.org/myhu and click on ‘Forgot Password.” Enter your email address and select “Send my username and password HINT,” or “Generate and send a new password.” Our computer system will send your request as soon as possible.

What if I reset my password, but the automatic email never arrives?
Some email programs will think our automatic email is span or junk, and will put it in your junk folder. If this isn’t the case, please notify myHUsupport@utah.gov and we will assist you.
Can I use my account to sign up my spouse for a testing session or submit a rebate for him/her?
No. Your spouse needs their own activated account to be able to sign up for a testing session, track/submit rebates, etc.
Why has my account been de-activated?
There are several potential reasons:
- If you have changed your insurance plan within PEHP, your eligibility then changes from inactive to active with the onset of the new plan.
- If you dropped your PEHP medical insurance coverage for any reason, your account becomes inactive.
- Your agency may have opted out of the Healthy Utah benefit recently.

My email address has changed. How do I update that on my account?
You can send your new email address to myHUsupport@utah.gov, or you can call our office at (801) 538-6261, or toll free at (888) 222-2542.
Why aren’t my testing session results posted on my account yet?
Occasionally we aren’t able to connect directly to the internet at a testing session, so your results are entered onto a laptop. Once the laptop is available in our main office, usually within one to two weeks, it is synchronized and your results will be online. Notify myHUsupport@utah.gov if your results aren’t posted after three weeks.

Why can’t I create a new account? The program says it can’t find me in the system.
Please make sure to register your new account with your first and last name exactly as it is shown on your insurance card. Also, use your 13-digit PEHP ID number (17410000xxxxx).
I submitted my rebate at least three weeks ago through my account, but I haven’t received a check yet. What should I do?
You can check the status of your rebates by logging into your myHealthyUtah account and clicking on “Rebate History.” The rebate status is updated as it is submitted for payment to PEHP. Once the status says “Paid,” your check should arrive within three to four weeks.

Can I still use myHealthyUtah after I retire?
Yes! Your myHealthyUtah account will remain active. You can also attend a testing session every year, although retirees are not eligible for rebates.

Rebate FAQs
What are rebates?
Healthy Utah wants to reward individuals for making changes that can improve health. Cash rebates are awarded to members who successfully accomplish specific criteria.
What rebates are offered?
Healthy Utah offers rebates in six different areas of health:
- Physical Activity - Individuals who are physically active for 100 days during the year receive $60 for their efforts. (No longer available after December 31, 2008)
- Tobacco Cessation - Current users can earn $100 by quitting tobacco for one year. Members may earn this rebate once only.
- Weight Loss - Up to $150 per year is awarded for improved body composition, measured by loss of pounds. Additionally, Healthy Utah will pay one-half the cost, up to $40, of an approved weight loss class.
- Reduced Cholesterol - Healthy Utah will pay $50 per year when high cholesterol levels are reduced a minimum of 10%.
- Reduced Blood Pressure - A $50 rebate is paid for reducing high blood pressure levels to within normal ranges.
- Diabetes Control - People with diabetes who maintain their HgbA1C at seven or less, or reduce it to that level, receive a $100 rebate.

Do I have to register for Healthy Utah before I can qualify for rebates?
No. If you are not currently a member, we will register you when you submit your rebate information.
Do I have to go through a testing session before I can start a rebate?
No. Testing sessions can be the easiest way to sign up for rebates, but you can participate in the rebate program without attending a testing session. You can download the Physical Activity Rebate and/or the Rebate Form and get started now.

How do I qualify for and participate in the rebate program?
Weight Loss
To qualify: Your body mass index (BMI) must be 25 or greater. To calculate your BMI, click here http://nhlbisupport.com/bmi, or attend a Healthy Utah testing session.
To earn the rebate: Lose 10 pounds and maintain that loss for six months to earn $60, then keep going for an additional $1 per pound, up to a total of $50. Once you have lost and maintained the first 10 pounds, receive another $40 by making specific behavior changes.
Required documentation: A non-family member (i.e. neighbor, coworker, friend, doctor) must verify weight loss, maintenance, and behavior changes on the Rebate Form. Remember to use the same scale to weigh yourself and to verify results.

Cholesterol
To qualify: Your total cholesterol level must be 200 mg/dL or higher. A valid cholesterol result can be obtained from a doctor's office or a Healthy Utah testing session.
To earn the rebate: Decrease your total cholesterol by 10%.
Required documentation: If results are from a physician, dated copies of pre- and post- cholesterol lab results must be submitted with your Rebate Form. Dated results must be within one year of each other. If results are from Healthy Utah, we will process your rebate at the testing session. Your health care provider or a Healthy Utah staff member must sign the Rebate Form.

Blood Pressure
To qualify: You must be diagnosed with high blood pressure by your physician. Healthy Utah staff are not qualified to diagnose this condition.
To earn the rebate: Working with your physician, set a goal blood pressure and successfully accomplish it.
Required documentation: Your physician must verify your original, goal, and final blood pressure results on the Rebate Form.

Diabetes
To qualify: You must currently be diagnosed with type I or type II diabetes.
To earn the rebate: You must lower or maintain your A1C at 7 or lower for the past year. The A1C tests reflects your blood sugar activity over a period of three months.
Required documentation: Recent A1C lab results from your health care provider must be submitted with your Rebate Form. Dated results must be within one year of each other. Healthy Utah does not have the equipment necessary to administer A1C tests.

Tobacco Cessation
To qualify: You must currently use tobacco, or have quit no more than 12 months prior to the rebate contract date. Once you have been quit for a year, simply complete the Rebate Form and submit it. You must use tobacco products for a minimum of one year before qualifying for this rebate, so it isn't worth it to start just to get the rebate!
To earn the rebate: You must be tobacco free for one year.
Required documentation: A non-family member (i.e. neighbor, coworker, friend, doctor) must verify that you have been tobacco-free for 12 months on the Rebate Form.

I attended a testing session about a year ago, but never turned in my rebates. Is it too late?
Once you sign a rebate contract, you have one year to complete it and earn as much money as possible. We prefer you to submit your rebates within that year, but will give you an extra month from the date on your contract to complete it (for a total of 13 months).

How long does it take to receive my rebate check?
Once we receive your rebate information, it can take up to six weeks for you to receive your check. Most checks arrive within a month, however. If it has been five weeks or longer since you submitted your rebate, please call our office.
Can my spouse participate in the rebate program?
Yes, if he/she is insured with PEHP.

Testing Session FAQs
What is involved in a testing session?
Healthy Utah offers testing sessions at various sites around the state. When you participate in a testing session, we will check your cholesterol, blood pressure, waist circumference, and weight. We also check your blood glucose if you are at risk for diabetes, or are 45 years of age or older. We then offer you one-to-one counseling and tips on how to improve your elevated test results. Attending a testing session is the easiest way to sign up for the rebate program we offer.

How long does a testing session take?
A complete session lasts about 20-30 minutes, depending on how busy we are. We generally schedule four people per half hour. If you have several questions or concerns, we are happy to spend extra time with you to ensure you have the information necessary to make informed and healthy choices.

Do I need an appointment?
Yes. An appointment is vital to ensure the quality of the health screening and to the schedules of those who attend. We value your time and want to make sure we help you in a timely manner.
Can I attend a testing session if I am pregnant?
We prefer you come to a Healthy Utah testing session at least three months after the delivery of your baby. This waiting period allows your cholesterol and body composition to return to baseline levels. Nursing will also keep your cholesterol level elevated, so use your best judgment when making an appointment to be tested.

Do I need to fast for the cholesterol or glucose tests?
No, you are not required to fast before coming to a testing session for the following reasons:
- Blood glucose test: A blood glucose test measures the amount of sugar present in your blood stream. At the testing session, we compare your glucose result with a non-fasting value.
- Cholesterol test: Total cholesterol is a value that takes weeks to change. Fasting before your cholesterol test will have little or no effect on the outcome of your result.
My cholesterol was normal at the last Healthy Utah testing session I attended. Should I have it tested again?
Medical guidelines recommend having your cholesterol checked once every five years if it is good (total cholesterol less than 200 or HDLs greater than 40). We will check your cholesterol once every two years if it was normal the year before. Cost of testing supplies prohibit us from checking cholesterol every year if your level is normal. You can still have your cholesterol tested, but will be charged a $10 fee.

If my blood pressure is elevated, do I have hypertension?
The American Heart Association refers to hypertension as a condition in which the resting blood pressure is consistently at or greater than 140/90 mm Hg (millimeters of mercury). A single elevated blood pressure measurement does not necessarily mean that you have hypertension, but it should not be ignored.
There are several reasons that multiple measurements are needed to diagnose hypertension. Instruments used to measure blood pressure are not always accurate, such as the automated cuffs available at pharmacies. Accuracy even varies among health care workers. There are also factors that can temporarily increase a person's blood pressure, such as taking over-the-counter cold medications or having an abnormally stressful day. For these and other reasons, any elevated reading found by someone other than your physician or their staff must be confirmed by your doctor.

Are the Healthy Utah staff registered nurses?
We are not nurses, but we are trained health professionals. Staff members either have or are working on Bachelor's degrees in Health Promotion and Education, Exercise and Sports Science, Nutrition, or other health-related degrees. Many of our team also have Master's degrees in health areas and other professional certifications.
Our staff members are required to pass stringent annual state certifications to perform cholesterol, glucose (blood sugar), and blood pressure tests. They are required to re-certify in First Aid and CPR on a yearly basis.

Wellness Seminar FAQs
What are Wellness Seminars?
Healthy Utah offers on-site seminars that focus on various health topics, including nutrition, physical activity, stress management, and personal/professional development. We also offer special wellness series, which are generally scheduled on a weekly basis over a period of two to twelve weeks. Wellness seminars are great for staff meetings, retreats, brown bag lunch-and-learns, or employee trainings. They are a great way to catch up on the latest health information.
How long do they last?
Most classes last about an hour, with the exception of the Color Code classes, which are about two hours each, and Simplify Your Life, which can run up to 90 minutes.

Is there a cost to participate?
Wellness Seminars are provided as a benefit to our members, so our classes are offered free of charge. The only exception is the Lighten Up! series, which costs $50 per person, $25 of which is refundable upon completion of nine of the twelve classes.
How can I attend the next seminar?
Healthy Utah does not offer "open" classes. Our seminars are scheduled upon request only. Scheduling a class is easy, and chances are if you are interested in a class, there are others at your worksite interested, too.
How do I schedule a Wellness Seminar?
Look over the list of seminars we offer. Once you have decided on the class(es) you would like to schedule, decide on your preferred date and time for the class, then contact Healthy Utah at HealthyUtah@utah.gov. She will check with the instructor for that class to determine availability and get the class scheduled for you.

How many people do we need to schedule a seminar?
Since Healthy Utah devotes a lot of time and resources to put our seminars together, we require a minimum of 15 people to schedule a class. We feel these classes are a great benefit to our members, and want to be able to share the information with as many people as possible. Sometimes the best way to get people to a class is to ask them which class(es) they are interested in before scheduling.
How far in advance should I call to schedule a seminar?
It is never too early to call us. We ask that you give us at least two weeks advance notice, but more is better.
